Only the parents or legal guardians can bring the child to the dental appointment. An ID will be required that matches the name of the parent or legal guardian at the day of the dental appointment or the appointment will be RESCHEDULE. If you are NOT the biological parent but are the legal guardian, please bring to the FIRST appointment the custodian papers otherwise your child appointment will be RESCHEDULE.

Families with more than one child that requires treatments may requires that for each child the appointment be scheduled on different days to avoid a long waiting period.
